The ability to provide simultaneous antegrade cerebral and systemic perfusion may negate the requirement for any prolonged period of circulatory arrest during complex aortic arch reconstruction. This versatile single pump-driven circuit offers a wide range of cannulation and perfusion strategies, is safe and simple to manage, and has now become the standardised approach for facilitating complex aortic work at our institution. This presentation details the progression of the circuit and the rationale for the changes to our practice.
